Why does urethral stricture come back after dilation, urethrotomy, or other urethral instrumentation?
Endoscopic treatments for urethral stricture (urethral narrowing) — including dilation and urethrotomy — were historically the only available options, but they are largely palliative rather than curative. They often create a temporary improvement in urinary flow, giving an illusion of success, while promoting further scar formation (spongiofibrosis) and recurrence.
Despite their simplicity and widespread use, repeated endoscopic treatments can worsen the disease. This is why modern management limits their use and prioritizes definitive solutions such as urethroplasty.
