Who we are

A Dedicated Unit for Advanced Urethral Reconstruction

This center is entirely dedicated to the diagnosis and treatment of urethral stricture disease.

Patients referred here often present with recurrent strictures after previous treatment and require urethral reconstruction (urethroplasty), including in complex cases or after multiple prior unsuccessful procedures.

Each case undergoes precise anatomical assessment in order to define a durable reconstructive strategy rather than temporary relief.

Comprehensive Diagnostic Work-Up

Evaluation includes history, examination, and targeted endoscopic and imaging studies to define the stricture.

Advanced Reconstructive Surgery for Durable Outcomes

Urethral stricture disease requires more than repeated endoscopic (endourethral) treatments such as dilation or direct vision internal urethrotomy (DVIU), which are associated with limited success and low long-term efficacy.

A structured reconstructive approach using urethroplasty—particularly with buccal mucosa grafts—aims to restore normal anatomy and urinary function while minimizing recurrence.

SPECIALIZED CARE

Experience in Complex & Recurrent Urethral Conditions

Urethral reconstructive surgery is a highly specialized field practiced by relatively few surgeons due to its technical complexity.

Professor Madec regularly manages:

– Recurrent strictures after urethrotomy
– Long bulbar or penile strictures
– Posterior urethral stenosis (VUAS, bladder neck contracture)
– Post-hypospadias adult reconstruction
– Lichen sclerosus–related disease
– Multi-operated complex cases

Each patient receives detailed counseling and a reconstruction-oriented plan focused on durable results.

01
Individualized Patient Evaluation
Individualized Patient Evaluation

Diagnosis of urethral stricture is based on several complementary steps, including symptom assessment, medical history, clinical examination, and patient-reported outcome questionnaires.

02
Advanced Urethral Imaging
Advanced Urethral Imaging

Dedicated investigations such as uroflowmetry, flexible cystoscopy, retrograde and voiding urethrography, CT urethrography, and urethral MRI help define the exact location, length, and severity of the stricture before treatment planning.

03
Endoscopic Treatments
Endoscopic Treatments

Selected short or first-time strictures may be managed with minimally invasive options, including urethral dilation, internal urethrotomy, and drug-coated balloon dilation (Optilume®), with appropriate discussion of recurrence risk.

04
Urethroplasty
Urethroplasty

Urethroplasty includes the full range of reconstructive techniques, from excision and primary anastomosis to buccal mucosa graft urethroplasty, perineal urethrostomy, and advanced redo urethral reconstruction, tailored to stricture characteristics.

05
Complex Cases
Complex Cases

Management of complex cases including post-hypospadias strictures, bladder neck contracture after BPH surgery, and post–prostate cancer stenosis (including vesicourethral anastomotic stenosis and radiation-induced strictures).

Why does urethral stricture come back after dilation, urethrotomy, or other urethral instrumentation?

Endoscopic treatments for urethral stricture (urethral narrowing) — including dilation and urethrotomy — were historically the only available options, but they are largely palliative rather than curative. They often create a temporary improvement in urinary flow, giving an illusion of success, while promoting further scar formation (spongiofibrosis) and recurrence.

Despite their simplicity and widespread use, repeated endoscopic treatments can worsen the disease. This is why modern management limits their use and prioritizes definitive solutions such as urethroplasty.
